We strongly encourage new patients to fill out the paperwork ahead of time and either email it to us (call us at 901-854-5527 for the email address) OR bring it with you to your appointment. If you are not able to print/fill out the forms, you may choose to arrive 15 minutes early for your appointment to fill out the paperwork in person.
